Ex-Offender Re-Enty
WHO IS ELIGIBLE?
To be considered eligible for admission into the Post-Release
Substance Abuse Transitional Housing Program, participants
shall meet the following criteria, as applicable.
A. Participant may be any recently released inmate from
Department of Corrections in need of transitional housing
upon release. However, the targeted population for this program
will be recently released with histories of substance use/abuse
problems, particularly those released inmates who have completed
a Department in-prison or community-based drug treatment
program who are in need of transitional housing services upon
release. Program participants do not have to be on community
supervision in order to be eligible for participation.
B. Participant must request to participate in a post-release
substance abuse transitional housing program.
C. Participant must meet the THORMINC’s Department-approved
eligibility criteria.
D. Participant must be in need of transitional services to promote
his/her successful reentry into the community, and abstain from
drug and alcohol use.
E. Participant must have a Community Supervision Referral Form
DC5-404 or electronic equivalent with Section I completed by the
Department.
F. Must not have any Disciplinary Write-ups within last 90 days
G. Must be willing to attend weekly Independent living classes
H. Must be tolerant and receptive of those who have different
religious views and practices
I. Must be willing to live well with others
J. Must be willing to give 3-6 months minimum of yourself to the
R.S.V.P. Program
K. Must attend AA/NA or some form of faith based church, temple
or synagogue
L. Must have determination to live right and not return to old
people, places and things
M. If one should relapse, must be willing to go into treatment
for disease.
N. Must be willing to live drug and alcohol free
When considering the admission of program participants,
THORMINC shall attempt to give first priority to participants
who have successfully completed a Department in-prison or
community-based outpatient or residential substance abuse
treatment program.

CASE MANAGEMENT
All participants of THORMINC’s Ex-Offender Re-Entry program
will be managed to help them outline their future goals and objectives
which will enable them to be self-sufficient,viable citizens of their
communities. At the completion of the program, all participants will
be assessed and successfully graduated upon completion. If you have
any questions concerning any aspects of case management, please feel
free to contact our office staff and arrange an appointment with our
Ex-Offender Re-Entry Department.
MOBILE RESOURCE RE-ENTRY CENTER
The mission: “Mobile Re-Entry Resource Center“ to provide a
comprehensive onsite job development and educational services to
benefit inmates at Florida DOC Facilities.
The Mobile Center will provide an onsite office environment consisting
of several laptop computers with supervised internet access, a printer,
cellular phone service and a shuttle service consisting of a minivan to help
employ those individuals serving at Work release Camps and transitioning
them back into the workforce or community in which they come from or once lived.

office at (904) 345.2873 for an appointment. Bring the completed application and
your on-the-job training certificates to your appointment for job review and/or referral.
Once the review has taken place, job training can be scheduled. Upon completion of
trainings you can be placed within you field of work.
